Currently, very few reasons prevent implant placement in a patient. The low quantity or poor quality of the bone is the biggest problem that can exist. Statistics show that dental implant placement is 98% successful in the first year and 90% successful in the first 10 years. Hundreds of thousands of implants are placed annually worldwide and are approved by all health authorities including the American FDA (Food and Drug Administration).
If there is proper hygiene and you see your dentist regularly, you will have no problems with the implants. There are many factors that can shorten the life of the implants, such as poor hygiene, genetic causes and the appearance of other diseases.
The implants are only placed after assessing the patient’s clinical situation using complementary diagnostic tests. In some cases the placement of the crowns can be done at the same surgical time as the implants.
If this protocol does not apply to you, it is necessary to perform the osteointegration of the implant, which will take about 2 months, and then the mold phase for the esthetic fixed prosthesis will begin. However, you can use a temporary prosthesis while healing the bone and gums. The total time, from the placement of the implants to the completion of the fixed prosthesis, can vary from 5 to 8 months, depending on the cases.
Treatment is carried out in an office and, in the vast majority of cases, with local anesthesia. During the intervention you will not feel any pain and, in the following hours, the pains are easily controlled with painkillers and the antibiotic prescribed by the dentist. The pains usually pass completely after a period of 2 to 5 days depending on the patient and the degree of the intervention.
The acceptance of dental implants by the human body is completely different from other cases, such as heart valve implants. In these cases, it is necessary to consider other factors such as tissue compatibility, interaction with blood factors, etc. that in dental implants are not so important. The adaptation of the human body to dental implants is based on a process called osteointegration in which the bone and the implant form a whole.
The placement of implants should be seen as a preventive measure and a medium-term investment. You should consider that dental prostheses damage the teeth on which they rest and cause bone wear which can make future implant placement more difficult or even impracticable. Placing implants before you start losing bone saves you money and increases the likelihood of success. What makes implant placement costly is the price of material that must be of high quality (not only the implants, but also the crowns) in addition to the study work, surgery and intermediate interim solutions.
The number of implants to be placed will depend on the dentist’s diagnosis. In general, the number of implants varies between 4 and 8 arranged according to the study that is carried out.
Lower removable prostheses are always more difficult to adapt. If you have enough bone, you can undoubtedly place a prosthesis on implants only in the jaw, keeping the upper removable prosthesis. In our dental clinic we do this work very often.
Types of Implant
A dental implant is a titanium cylinder with a threaded surface that is placed inside the jaw bone and that will work as if it were the root of our teeth. The implants are placed under local anesthesia, without the need for general anesthesia, and the surgery is performed in a sterile environment to prevent infections.
As a general rule, implants are placed without incision in the gum and without stitches. A 4 mm diameter hole is opened in the bone and the implant is placed there. Then, a healing cap is screwed onto the implant and the temporary crown is placed.
However, these obstacles can be overcome with techniques to increase the amount of bone using synthetic material, bio-bio, the most frequent technique of regeneration, or using the patient’s own bone (chin, jaw or iliac crest).
